The New York State Appellate Division courthouse on Monroe Pl in Brooklyn has a bunch of wooden phone booths.
Art Student's League on 57th Street has two
I know the Broadway branch of the Queens Library used to have a few wooden phone booths until I was in college, which alas, was 20 years ago. I haven't been to that branch since. I do know they "renovated" recently, so they may have been removed.
There are wooden phone booths in the lobby of New York Hospital - 525 East 68th Street.
There are some great ones inside the lobby of 31 Chambers Street, Surrogates Court.
The Eight Avenue side of the Park Slope Armory (Between 15th and 16th Street, Brooklyn, has two of them.

Another wooden phone booth sighting lies in the circa 86 year-old walls of Eddie's Sweet Shop at 105-29 Metropolitan Ave in Forest Hills. Copy & Paste this link into your browser, as well as view the other photos of one of the last classic ice cream parlors citywide:

The University Club has numerous wooden phone booths.
There used to be one inside the Film Center Cafe on 44th and 9th in Manhattan when I used to go there in the 90s. Don't know if it's still there. I would think so.
Liedy's Shore Inn, Staten Island
I recently scouted the Appellate Division of the NY Supreme Court-
45 Monroe Pl. Was thrilled to see the wooden telephone booths. Also, I'm pretty certain Ear Inn, Tribeca are on West side has one.
Great site! Thanks.

The Research Library of the New York Public Library on 5th and 42nd St. has a number of them on different floors. The ground floor and the first floor I think. They are on the north (42nd St.) side of the building at the foot of the marble staircases.
Montero's on Atlantic has two non-functioning ones.
The beautiful wooden booth on the way to the ladies' room at The Frick.
NYCLA (New York County Lawyers Assoc.) has more than one in the library.
